1. I am a financial member of the Burleigh Heads Mowbray Park Surf Life Saving Club,
2. all personal details contained is this document are confidential and not for distribution,
3. are for Surf Life Saving related use only,
4. will not use any information contained within to harass, menace, discriminate or unfairly treat any other member,
5. will take all means possible to protect this information once in my possession,
6. will not forward, copy or pass these details on to anyone (including email, paper or web based),
7. will destroy this information if I am no longer entitled to hold a copy.

I understand that should I not abide by any of the conditions that I may be required to appear before the Club Membership, Judiciary or Management Committees and my actions may be regarded as criminal and referred to the appropriate authorities.

For the safety and well being of all members of the Burleigh Heads Mowbray Park  Surf Life Saving Club, I will "do the right thing".
